Summary I n the care of patients with keratoconus, considerable hardship is sometimes imposed by delay in reaching the decision to operate. Contact lenses may provide immense benefit, but it is essential to know when they should be abandoned in favour of keratoplasty. The management of keratoconus should, therefore, be directed by an ophthalmic surgeon who is familiar with the recent advances of contact lens fitting and also with modern techniques of keratoplasty.
