S ummary —A bstract A case of tuberculous dacryocystitis in a 78‐year‐old woman is described. It presented with recurrent episodes of dacryocystitis over many years. A skin lesion arising from the site of fistula formation was diagnosed at one stage as a basal cell carcinoma but histological examination and culture provided the diagnosis of tuberculosis of the skin and histological examination of the extirpated sac showed tuberculosis in its wall. This case is presented because of the extreme rarity in present‐day practice of this condition, namely infection of the lacrimal sac by Mycobacterium tuberculosum.
