Chemotherapy in the acute stage of urinary tract infection in children in Japan, is discussed in regard to the actual state of major causative organisms and the drug susceptibility. The most important thing in the selection and administration of the drug used is to have a good understanding of severity and course of the patient's illness, his state of renal function, presence of underlying disease, etc., and a good knowledge of the characteristics of various drugs as well as the differences between them. In most cases, it seems proper to start the initial treatment by selecting those drugs which are considered appropriate from among the first generation cephems, the second generation cephems and synthetic agents, and in very severe cases, from among the second generation cephems or the third generation cephems, and to change to the most suitable drugs when the results of drug susceptibility tests become known.
