Spindle cell lipoma is an unusual and special type of lipoma which has been occasionally misdiagnosed as liposarcoma by histopathologic examination. Twelve patients with this benign subcutaneous tumor were selected from about 8,000 cases of soft tissue tumors in our files, and the clinical and pathologic features of this tumor were reviewed and analyzed. The lesion exclusively affected adult males, and the nuchal region was the site of predilection. Microscopically, it is composed of an admixture of mature fat cells and fibroblastic spindle cells, accompanying collagen and reticulin fibers interspersed in a rich mucoid substance. The follow‐up information of the patients has revealed a benign clinical course.
