An autopsy case of aleukemic megakaryocytosis developing in a 60‐year‐old farmer Is reported. This case was clinically suspected of aplastic anemia and took a rapid fatal course of approximately 3 months. Autopsy revealed a special type of myeloproliferative disorder characterized by diffuse proliferation of unclassified mononuclear blast cells and their selective differentiation toward megakaryocytes, although they were mostly atypical in their appearance and immature in their productive potency of platelets. Difference from myelofibrosis with myeloid metaplasia was also discussed.
