An autopsy case of a 34‐year‐old male who had been suspected of having rheumatic myocarditis with eosinophilia was described. At autopsy the heart was remarkably enlarged (780 g) with mural thrombi in both ventricles. Histology revealed features of so‐called fibroplastic parietal endocarditis with eosinophilia or Ldffler's endocarditis. Vascular changes, mainly of arterioles and consisting of endothelial proliferation, thickened intlma, and rarefaction of media were observed in nearly all organs. The etiology and pathogenesis of this condition are at present unknown.
