An autopsy case of a 25‐year‐old nulliparous woman who died of acute fatty liver of pregnancy (the so‐called obstetric acute yellow atrophy of Sheehan) was reported. Histology revealed swollen liver cells having droplet‐like or foamy appearance due to intense fatty infiltration. There was no massive necrosis or inflammatory reaction. The available cases of this disease in the literature were reviewed, and some discussions were made on its pathogenesis.
