Summary The behavior of the pollicomental reflex was observed in relationship to the site and nature, and the progress of cerebral lesions as demonstrated by arteriography or findings at operation. The reflex is a usuful diagnostic sign for intracranial lesions as it often reveals organicity of the disease and indicates the correct side (for arteriography, etc.) in early stages in which the lesion cannot be detected by other clinical means. The reflex is usually positive in lesions in which abnormality is found in the area supplied by the middle cerebral artery. However, it may later disappear in few cases in which the lesion is destructive to the central area presumably due to the interruption of the reflex pathway.
