Abstract A case of HIV seroconversion associated with high viral load occurring at or near delivery is described. The management of the case in terms of prevention of mother‐to‐child‐transmission is described and discussed in terms of the published literature and guidelines. From this instructive case a constructive strategy for delivery and immediate post‐natal care is derived.
