Hypogonadism in a 17‐year‐old boy who received cyclophosphamide at the age of 8 years is described. He had nephrotic syndrome with focal sclerosing glomerulopathy. He received cyclophosphamide for 96 days, total dose of 7200 mg or 240 mg/kg, which is less than 3 mg/kg/day. It is suggested that his hypogonadism is a direct result of cyclophosphamide therapy. The response of the prepubertal gonad to this drug appears to be highly variable.
