SYNOPSIS The case histories of 23 neonates with haemoperitoneum in the first week of life are reviewed. Ruptured liver was the most common cause (70%) and in this group survival was more related to prompt recognition and resuscitation than to whether laparotomy was performed or not. The volume of blood lost was large and often underestimated. Their characteristic mode of presentation is contrasted with those with haemorrhage from other organs. No matter what the cause, resuscitation is needed as an emergency. Careful clinical, radiological and laboratory assessment should then reveal the diagnosis and determine the need for further treatment and especially laparotomy.
