SYNOPSIS The clinical details of a 14‐month‐old girl, presenting with hypoproteinaemia, generalized oedema, steatorrhoea, hypocalcaemia and hypoprothrombinemia are described. Malrotation of the gut was diagnosed by barium radiographic studies. At laparotomy, chronic volvulus causing obstruction of the mesenteric lymphatics and veins was demonstrated. Protein‐losing enteropathy associated with lymphatic obstruction is considered the most likely cause of the presenting features. A milk containing medium chain triglycerides instead of long chain fats was given pre‐operatively and although other therapeutic measures were also employed an analysis of the clinical progress suggests that this feeding was of value and its use is worthy of consideration in patients with protein‐losing enteropathy.
