This paper reviews the basic procedure for designing a data collection form that requires little additional time but provides data useful for clinical records as well as research purposes. Treatment goals are evaluated by quantifying patient behaviour during activities related to the goals. Measurement should require little or no extra equipment. Coding should be fast and easy and able to be described on the form. The format should permit rapid recording and analysis of data. The process for developing such a form is described step by step. Problem areas are reviewed and solutions proposed.
