SUMMARY The chance seeing of a case of pili annulati provided an occasion for reviewing the salient features of this condition. Previous observation that the abnormal areas disappear after exposure to weak alkali on a glass microscopic slide was confirmed. Electron microscopic examination of hair samples from the young man and his elder sister suggests that there is a primary defect in the synthesis of the microfibril + matrix complex with possibly less of it being formed and thus creating a potential space. The process appears to be intracellular. The gas in the canals or spaces is likely to be air due to the fact that hair fibres are relatively permeable to most gases and liquids.
