Treatment of Onychomycosis with a Propylene Glycol‐Urea‐Lactic Acid Sohtion Onychomykose‐Behandlung nit einer Propylenglykol‐hamstoff‐Milchsäure‐Lösung

Summary: Twenty three patients with culture positive onychomycosis were treated topically twice daily for 2–6 month with a solution containing 10 g urea, 15 g lactic acid, 4 g NaOH, 21 g water, and 50 g propylene glycol. Trichophyton rubrum was cultured in 17 patients and Candida albicans in 6 patients. The solution was effective in 21 of the 23 patients with cure in 2 patients, marked improvement in 11 patients and improvement in 8 of the patients. In the patients with marked improvement some nails were often completely cured. The good effect of this solution in the treatment of onychomycosis is in correlation with a good in vitro activity with MICs of 5 % against Tr. rubrum and 10 % against C. albicans and a high penetration into nails in vitro. The patients found the solution cosmetically attractive and no side effects were noticed by the patients. Even if the treatment period was short the good effect in the majority of the patients indicates that this solution is effective in topical treatment of onychomycosis. Zusammenfassung: Dreiundzwanzig Pa‐tienten mit kulturell positiver Onychomykose wurden zweimal täglich 2–6 Monate lang topisch mit einer Lösung aus 10 g Harnstoff, 15 g Milchsäure, 4 gNaOH, 21 g Wasser und 50 g Propylenglykol behandelt. Trichophyton rubrum wurde bei 17 Patien‐ten und Candida albicans bei 6 Patienten kulturell nachgewiesen. Die Lösung war bei 21 von 23 Patienten wirksam mit Heilung bei 2 Patienten, deutlicher Besserung bei 11 und Besserung bei 8 Patienten. Bei den Patienten mit deutlicher Besserung wurden ofter einige Nägel vollständig geheilt. Die gute Wirksamkeit dieser Lösung bei der Onychomykose‐Behandlung korreliert gut mit der Aktivität in vitro mit MHK von 5 % gegen Tr. rubrum und 10 % gegen C. albicans und mit der hohen Penetration in Nägel in vitro. Die Patienten beurteilten die Lösung als kosmetisch angenehm, Nebenwirkun‐gen wurden bei den Patienten nicht beob‐achtet. Die guten Behandlungsergebnisse in der Mehrzahl der Patienten selbst bei kurzer Behandlungsdauer belegen, daß die Lösung bei der topischen Behandlung der Onychomykose wirksam ist.
