Management of Combined Bacterial and Fungal Foot Infection in Coal Miners

Summary: Therapy of foot infection in coal miners is difficult, partly due to their acceptance of the inevitability of its occurrence and partly due to the presence of mixed infection. Topical antifungal drugs (clotrimazole, ketoconazole) suppressed dermatophytes in 55% to 72% of men but led to an increase in Gram‐negative bacilli in some instances, whilst suppression of Gram‐negative bacilli may lead to the appearance of dermatophytes. Mixed fungal/bacterial infection of the toe‐ webs in coal miners is an important cause of morbidity in the industry. It is necessary to recognise that control of foot infections in this group may require measures which will combat both fungi and Gram‐negative bacteria. Zusammenfassung: Die Therapie der Fußinfektion bei Bergleuten ist schwierig, einmal da sie als unvermeidlich hingenommen werden, zum andern wegen des Auftretens von Mischinfektionen. Topische Antimykotika (Clotrimazol, Ketoconazol) unterdrückten Dermatophyten in 55–72% der Männer, führten aber in einigen Fällen zur Zunahme gramnegativer Stäbchen, während die Unterdrückung der gramnegativen Stäbchen zum Auftreten von Dermatophyten führen kann. Pilz‐Bakterien‐Mischinfektionen im Zehenzwischenraum von Bergarbeitern ist eine wichtige Krankheitsursache in diesem Industriezweig. Es ist notwendig, darauf hinzuweisen, daß die Bekämpfung von Fußinfektionen in dieser Patientengruppe Maßnahmen verlangt, welche sowohl die Behandlung von Pilzen als auch die von gramnegativen Stäbchen umfassen.
