Lupus Erythematosus like Tinea Capitis caused by Trichophyton Tonsurans

Summary: Tinea capitis and tinea facei resembling chronic discoid lupus erythematosus (CDLE) caused by Trichophyton tonsurans in an adult female is described. Tinea corporis of the waist resembling lichen planus (LP) and tinea unguium due to the same organism were also observed in her. Hypopituitrism was the associated disease and long‐term oral griseofulvin and topical miconazole followed by econazole cream were necessary to eradicate the disease. The possibility of dermatophytosis should be kept in mind in cases of clinically diagnosed CDLE and LP in the tropics. Zusammenfassung: Bericht über eine Patientin mit Tinea capitis und Tinea faciei. Die durch Trichophyton tonsurans verursachte Dermatophytose ähnelte einem Lupus erythematodes chronicus discoides. Außerdem fanden sich bei der Patientin eine Onychomykose sowie eine Tinea corporis an der Hüfte, verursacht durch den gleichen Pilz, mit Anklängen an einen Lichen ruber planus. Gleichzeitig bestand eine Hypophysenunterfunktion. Erst nach langfristiger Behandlung mit oralem Griseofulvin und örtlicher Anwendung von Miconazol und Econazol konnte die Mykose ausgeheilt werden. Bei Patienten mit klinischen Zeichen eines Lupus erythematodes chronicus discoides oder eines Lichen ruber planus in den Tropen sollte die Möglichkeit einer Dermatophytose differentialdiagnostisch erwogen werden.
