Perkutane Resorption von Isoconazol beim Menschen: Percutaneous Absorption of Isoconazole in Humans

Zusammenfassung: Die Resorption von Isoconazol, von Diflucortolon‐21‐vaIerat (DFV) und von Fluocortin‐Butylester (FCB) wurde nach epikutaner Applikation von 4 verschiedenen Zubereitungen (A. Creme mit 1% Isoconazolnitrat, B. Creme mit 1% Isoconazolnitrat und 0,1% Diflucortolon‐21‐valerat, C. Creme mit 1% Isoconazolnitrat und 0,75% Fluocortin‐Butylester, D. Lösung mit 1 % Isoconazolnitrat) durch die intakte und “gestrippte” Rückenhaut an je 3 Probanden anhand der Radioaktivitäts‐ausscheidung mit Harn und Stuhl untersucht. Aus den Zubereitungen A, B und C wurden 3 H‐Isoconazol, 3 H‐DFV und 3 H‐FCB selbst durch die Haut, bei der die Hornschicht durch “Stripping” mit Tesafilm entfernt war, nur in sehr geringem Maße perkutan resorbiert. Doch war die Resorption deutlich höher als nach Applikation auf die intakte Haut. Insgesamt gelangten während einer mittleren 4stündigen Einwirkungszeit auf eine gleiche Anzahl intakter und “gestrippter” Hautfelder weniger als 1% der appl. Dosis in den Organismus. Nach Applikation der Lösung D lag der resorbierte Anteil mit ca. 5% der appl. Dosis deutlich höher. Summary: Absorption of isoconazole, diflucortolone‐21‐valerate (DFV) and fluocortin butylester (FCB) through intact and “stripped” skin on the backs of groups of 3 subjects for each substance was studied by measuring the radioactivity eliminated with the urine and faeces after epicutaneous administration of 4 different formulations (A. cream containing 1% isoconazole nitrate, B. cream containing 1% isoconazole nitrate and 0.1 % diflucortolone 21‐valerate, C. cream containing 1 % isoconazole nitrate and 0.75 Vo fluocortin butylester, D. solution containing 1% isoconazole nitrate). Percutaneous absorption of 3 H‐isoconazole, 3 H‐DFV and 3 H‐FCB from formulation A, B and C was very limited even in respect of the skin from which the stratum corneum had been removed by “stripping” with adhesive tape (Tesafilm). However, this absorption was noticably higher than that after application to the intact skin. In all, less than 1 % of the administered dose entered the body in the course of an average 4‐hour period of exposure on an equal number of intact and “stripped” sites. After administration of solution D the proportion of the dose which was absorbed was distinctly higher at 5%.
