Summary. Intensive plasmapheresis, between 15% and 24% weeks of gestation, of a highly iso‐immunized Rh‐negative woman, with the removal of 46 litres of plasma did not reduce her Rh antibody titre. It is unlikely that plasmapheresis ameliorated the degree of disease or influenced the ultimate survival of the very severely affected fetus. Intensive plasmapheresis would appear to be of little value in the management of severe Rh iso‐immunization.
