Über einen Fall von Morbus haemolyticus neonatorum, wahrscheinlich bedingt durch Antikörper des Lewis ‐Blutgruppensy stems

Summary A case of haemolytic disease of the newborn probably due to anti‐Le a and anti‐Le b antibodies is described. It was the woman's first pregnancy. The child was successfully treated by exchange transfusion. In the course of the following days the haemoglobin level decreased gradually. A transfusion of compatible Le(a‐b‐) blood was given; the donor, however, had not been tested for the presence of Lewis substance in the saliva. A severe reaction (collapse) followed. A following transfusion with Le(a‐b‐) blood, the donor of which proved to have no Le b substance in the saliva, was well tolerated. Résumé Rapport d'un cas de maladie hémolytique du nouveau‐né (mère primipare) probablement causée par des anticorps anti‐Le a et anti‐Le b . Au cour du traitement, une transfusion de groupe Le(a‐b‐), dont le donneur, cependant, n'avait pas été éxaminé pour la sécrétion de substances Lewis , causa une réaction transfusionnelle grave. Une transfusion ultérieure de sang Le(a‐b‐), dont le donneur n'avait pas de substances Le b dans sa salive, fut tolérée sans incident. Zusammenfassung Es wird über einen Fall von Morbus haemolyticus bei einem Neugeborenen einer Primipara berichtet, der wahrscheinlich durch Anti‐Le a + und Anti‐Le b ‐Antikörper verursacht wurde. Im Verlaufe der Behandlung bewirkte eine Transfusion von gruppengleichem Le(a‐b‐)‐Blut, dessen Spender jedoch nicht auf die Ausscheidung von Lewis‐Substanzen im Speichel geprüft w‐urde, eine schwere Transfusionsreaktion. Eine später durchgeführte Transfusion von Le(a‐b‐(‐Blut, dessen Spender keine Le b ‐Substanz im Speichel ausschied, wurde reaktionslos vertragen.
