Peak Expiratory Flow During a Passive Exhalation: An Index of Expiratory Resistance During Anesthesia

During a passive exhalation performed between (FRC + V T ) and FRC, peak expiratory flow (Vmax E ) varies as a function of the respiratory system expiratory resistance, R T , with the following relationship: Vmax, = V T /(R T C T ), where C r is the total respiratory system compliance (static). This expression assumes: (1) a laminar flow regimen during expiration, and (2) a constant C T value over the range of V T . Both these conditions arc usually attained during controlled ventilation in the apneic patient. Vmax E can be monitored continuously with relative ease, and may represent a simple and useful index of the mechanical properties of the respiratory system in the anesthetized patient.
