The Clinical use of a New Nerve Stimulator

SUMMARY The potential uses of a compact, light, battery‐operated nerve stimulator are presented. It can be used in the operating room as a clinical device, as a teaching aid and for purposes of investigation. The nerve stimulator is a useful monitor for the continuous control of muscle relaxation during surgical procedures. It provides a reliable and harmless way of titrating the need for skeletal muscle relaxant and gives valuable information on the effects of anesthetic drugs and techniques on abdominal relaxation. It is particularly useful at the end of surgery when inadequate ventilation is present. It allows the rapid and precise differentiation between central and peripheral types of apnea. Finally, it determines the type of neuromuscular block and evaluates the reversal of the block by anticholinesterase agents. The nerve stimulator has a definite place among the standard monitoring equipment of the clinical anesthesiologist. ZUSAMMENFASSUNG Es wird über die Anwendungsmöglichkeiten eines tragbaren, leichten, batteriegespeisten Nervstimulators berichtet. Er kann im Operationssaal als klinisches Hilfsmittel, als Lehrbehelf und für Forschungszwecke verwendet werden. Der Nervstimulator ist ein brauchbares Überwachungsgerät für die kontinuerliche Kontrolle der Muskelerschlaffung während operative Eingriffe. Er gestattet die verlässliche und harmlose Titrierung der erforderlichen Mengen von Muskelrelaxantien und gibt wertwolle Informationen über die Wirkung der Agentien und Techniken der Narkose auf die Bauchdeckenentspannung. Besonders nützlich ist das Gerät am Ende der Operation beim Vorliegen inadäquater Ventilation. Es gestattet die rasche und genaue Differenzierung zwischen zentraler und peripherer Atemlähmung. Schliesslich kann man damit den Typus des neuromuskulären Blocks bestimmen und die Wirkung der Antidote bewerten. Der Nervstimulator hat einen festen Platz im Armamentarium des klinischen Anaesthesiologen.
