The World Health Organization (WHO) and the lnternational Society of Hypertension (ISH) periodically issue guidelines for the management of mild hypertension. A third revision of these guidelines has been finalized recently and is being published in journals specifically devoted to hypertension. This paper presents a condensed version of these guidelines, so that the essential message can reach the wider audience of practising physicians who are responsible for the care of hypertensive patients. The present guidelines for the management of hypertension concentrate on mild hypertension as this condition often presents a diagnostic and therapeutic problem, and evidence for benefit in treating more severe hypertension is clear. In this revision emphasis is also placed on systolic blood pressure as a criterion for decision making, and on hypertension in the elderly-another condition in which careful judgement is necessary.
