Summary Objective: To estimate the effect of demography, deprivation and ethnicity on variations in prescribing of oral antidiabetics at the Primary Care Trust level. Design: Cross‐sectional survey. Setting: All Primary Care Trusts in England. Main outcome measures: Percentage of variance explained. Results: A model using measures of demography, ethnicity, deprivation and use of thiazolidinediones explained 59% of the variation in cost per registered patient although individual variables explained far less. Conclusions: This study shows the importance of using a multivariate approach in modelling the drivers of cost. The high use of thiazolidinediones is of concern in the light of NICE guidance.
