SUMMARY An investigation into adverse reactions to drugs arising in a hospital over a 5 week period is described and the role of the ward pharmacist in detecting and reporting those reactions discussed. Sources of information available to confirm a diagnosis of ‘adverse drug reaction’ are described and the problem of establishing clinical significance examined.
