Summary The present common clinical uses of radiopharmaceuticals are described and the reasons for the importance of those labelled with short‐lived radionuclides, in particular with technetium‐99m, are explained. The development in the practice of hospital preparation is traced from the first availability of long‐lived radiopharmaceuticals to the introduction of shortlived preparations. These latter were first made by in‐house methods and then with the aid of commercial kits. The results of the application of good manufacturing practice guidelines to radiopharmaceutical production are examined, in particular to the provision of safe facilities for preparation and to standards of quality assurance for the products.
