Summary. The use of endodontic surgery enhances the retention of many teeth which would otherwise be extracted. This applies to the posterior region as well as the anterior region of the oral cavity. However, because of variations in regional and radicular anatomy in the posterior, and the need to establish adequate access through proper flap design in a more confined area, it is necessary that the practitioner should have a thorough understanding of the principles of posterior endodontic surgery.
