Examination of the base of ulcerated gastric carcinomas in Okinawa, Austria and the USA revealed a variable infection with fungi and a thus far unidentified filamentous bacterial organism. These latter organisms were only found in ulcerated carcinomas, and never in diffuse carcinomas of the stomach or in benign gastric lesions. Recognition of these organisms in necrotic material from gastroscopic biopsies of an ulcer base without tissue from the ulcer margin permits the diagnosis of associated carcinoma.
