Summary Giant congenital pigmented naevi and neurofibromatosis (NF‐1) may rarely occur together. We report an unusual case where extensive congenital melanocytic naevi were associated with neurofibroma‐like lesions that were clinically and histologically confused with neurofibromatosis. The development of malignant melanomas within the pigmented and pendulous lesions representing multiple congenital melanocytic naevi highlights the importance of an accurate diagnosis and a close follow‐up of such patients.
