Summary A patient presenting with cutaneous vasculitis was noted to be pancytopenic. Subsequent investigation showed a myelodysplastic syndrome (MDS)–refractory anaemia with excess of blasts. Within 3 months her MDS progressed to acute myeloid leukaemia.
