Summary We report a case of adult onset, unilateral, progressive, grey‐blue pigmentation on the back of an Asian male. Histology showed the pigmentation to be due to deep dermal melanocytes. The spectrum of clinical presentations of dermal melanocytosis is reviewed and the unique features of this case outlined.
