Summary Two siblings suffering from the so‐called twenty‐nail dystrophy are described. This syndrome is characterized by the appearance in early childhood of changes affecting all the nails: moderate koilonychia, marked accentuation of longtitudinal ridges and loss of lustre of the nail plates. These two cases, the first reported in the same family, confirm that the twenty‐nail syndrome is a clinical entity which can be differentiated from other diseases involving all twenty nail plates.
