Summary Ingrowing toe‐nails in children and adults cause a considerable degree of morbidity. Much has been written about the treatment but very little attention has been paid to the basic anatomical and pathological factors causing the phenomenon. We wish to report thirty cases in which congenital malalignment of the nail apparatus is the basic abnormality. In this condition the nail plate is deviated laterally with respect to the longitudinal axis of the distal phalanx. We have devised a simple operation which corrects the deformity and prevents subsequent ingrowing and the hemionychogryphosis which is often the long‐term problem in untreated cases.
