Summary Dizygotic twin brothers affected by both ichthyosis and hypertrophic pyloric stenosis are described. Familial and biochemical studies demonstrated that the ichthyosis was X‐linked with deficiency of steroid sulphatase. This observation suggests that an X‐linked recessive gene may operate with other genetic and environmental factors in the aetiology of hypertrophic pyloric stenosis.
