Summary A case of malacoplakia of the vulval skin is reported in a patient receiving systemic steroids and immuno‐suppressive drugs for severe rheumatoid arthritis. The microscopic abnormality of this lesion consisted of a granulomatous infiltrate composed of histiocytic cells rich in PAS positive granules with inclusions known as Michaelis‐Guttmann bodies. The cells were embedded in a scanty connective tissue stroma infiltrated by lymphocytes and plasma cells.
