S ummary . In 12 patients with idiopathic haemochromatosis and varying degrees of iron overload the serum ferritin concentration was related to the amount of iron mobilized by venesection. There was no correlation between iron load and chelatable iron. In 25 patients with normal iron stores following venesection therapy, both transferrin iron and chelatable iron was pathologically increased. The initial stages of iron accumulation in idiopathic haemochromatosis are characterized by an imbalance between chelatable iron and ferritin, a phenomenon that could explain the pathogenesis of the disease.
