Summary. Patients with untreated pernicious anaemia usually show the following morphological and biochemical abnormalities in small‐intestinal biopsy specimens: (1) shortening of villi with diminished surface area; (2) megalocytic changes of the epithelial cells; (3) increased inflammatory infiltrate; (4) generalized depression of disaccharidases. These abnormalities revert to normal following treatment with injections of vitamin B 12 .
