Annular vasculitis of the head and neck in a patient with sarcoidosis

SUMMARY A patient with bilateral hilar lymphadenopathy due to sarcoidosis presented with erythema nodosum on the shins and upper arms and an unusual annular eruption resembling erythema annulare centrifugum on the head and neck. Biopsy of the latter showed a leucocytoclastic angiitis. The aetiology of sarcoidosis is unknown. There is increasing evidence for the presence of circulating immune complexes in the early stages of acute sarcoidosis which may present as erythema nodosum, iritis, polyarthralgia and bilateral hilar lymphadenopathy (James, 1978). Circulating immune complexes may result in a localized cutaneous vasculitis (Ryan & Wilkinson, 1979).
