SUMMARY A 35‐year‐old woman developed ischaemic changes over the soles of her feet within hours of admission to hospital moribund in hyperglycaemic coma. Skin and subcutaneous tissue separated but slow primary healing with scar tissue has taken place. An explanation for the plantar gangrene is offered.
