SUMMARY The term sporadic ulcero‐osteolytic neuropathy is used to describe isolated cases with the triad of peripheral neuropathy of the legs, ulcers, and painless destruction of bones and joints of the feet. Seven such cases in male South African Bantu are presented, in whom the changes were confined to the feet. Common causes such as leprosy, tabes dorsalis and diabetic neuropathy were excluded. Although a single cause could seldom be blamed, chronic alcoholism, malnutrition, and local trauma appeared to contribute. The clinical picture differed somewhat from other reported cases of similar causation. Sural nerve biopsy demonstrated a nerve lesion other than leprosy.
