Summary Attention is directed to the clinical recognition of early proliferative change in the lining of keratinous cysts. In the case reported, multiple small droplet excrescences could be observed on simple inspection. The term hydatidiform keratinous cyst is proposed as a designation for this distinctive clinical variant. Histologically it is to be distinguished from squamous cell epithelioma. It possibly represents an early clinically identifiable phase of the proliferative epidermoid cysts described by Wilson Jones (1966).
