Summary A case of life threatening airway obstruction secondary to bronchial compression following right pneumonectomy is reported. The aetiology of postpneumonectomy syndrome and the anaesthetic dilemmas faced are discussed with reference to the adaptation and use of a chest drain tube as an improvised airway to bypass the bronchial obstruction.
