Summary A case is reported of survival following accidental cannulation of the innominate artery during cardiopulmonary bypass. Coldness of the right side of the neck was noticed during cooling of the patient and is a useful new sign of misplacement of the aortic cannula. Confirmation of miscannulation of first the innominate then the left common carotid artery was obtained by palpation of the neck pulses during total cardiopulmonary bypass using a pulsatile pump.
