Post-operative chest infections due to colifonns of the Bacterium, Proteus and Psezcdomonas groups are a well recognised complication of heart surgery. This is particularly so in patients maintained on ventilators for long periods. In August 1965 we saw two such patients. One developed bilateral lung abscesses due to E. coli; the other had an empyerna caused by Ps. pyocjanea. The infection was a major cause of death in both. In neither, could we discover the source of infection. In addition, we had noted on several occasions, that Gramnegative bacilli were present in the sputum of patients on the first and subsequent post-operative days although clinical infections did not ensue. These events stimulated the following investigation into the origin of such infections or colonisations.
