Abstract Lucio's phenomenon is infrequent in our experience. Only 2 young men on multi‐drug therapy were seen among 36 erythema nodosum leprosum reaction cases in the last four years. It first appeared as extensive crops of puerpuric macules which transformed into dome‐shaped polygonal pustules and multiangular, bizarre, punched out ulcers. Necrotic greyish‐white debris was prominent the histological features were diagnostic. Multi‐drug therapy seemed to have instigated the process. The classical as well as the alterative pathway of complement activation is incriminated in its probable immune mechanism.
