A 74‐year‐old female patient with sarcoidosis who had cutaneous lesions on the scalp and lower legs, cervical lymph node enlargement, and bilateral hilar lymphadenopathy was studied. Histologically, aggregates of foam cells intermingled with lymphoid cells in addition to epithelioid cell granulomas were detected in the atrophic plaque with yellowish macules of the scalp. Additionally, she had brownish nodules on the lower legs with degenerative changes of connective tissue surrounded by epithelioid cell granulomas. In this degenerative area, irregularly clumped elastic fibers were observed. These elastic fibers were stained positively by the von Kossa method.
