Hidroacanthoma simplex is a rare tumor and its histogenesis and classification have been the subject of discussion, especially as to its relation to eccrine poroma. The tumor in this study developed on the knee of an old woman as a Bowen's disease‐like lesion. Clinical and histological examinations confirmed the diagnosis of hidroacanthoma simplex. Ultrastructurally, the tumor cells revealed a structure similar to that of eccrine poroma as regards numerous glycogen granules and the ring‐shaped distribution of tonofilaments, but different from eccrine poroma in having no intracellular cavity formation and underdeveloped tonofilaments and desmosomes. These findings indicate that hidroacanthoma simplex is related to eccrine poroma, but is not identical to it nor an intraepidermal form of it.
