Hyperosmolality in Leptospirosis

Summary A case of leptospirosis with renal and hepatic involvement developed hyperosmolar coma while undergoing peritoneal dialysis. The generous use of hypertonic (4.25% glucose) dialysing fluid combined with uraemia and liver dysfunction were probably important aetiological factors. Such factors commonly occur in severe leptospirosis. More‐over, the patient had an abnormal cortisone‐glucose tolerance test although his glucose tolerance test was normal. The development of marked diuresis during and immediately following dialysis as occurs in the case described would suggest the presence of severe hyperglycaemia with hyperosmolality.
