SUMMARY (1) Our experiences with 20 cases of emergency porta‐caval shunt for bleeding cesophageal varices are reviewed. (2) The difference in behaviour between cryptogenic and alcoholic cirrhotics is outlined, and the need for strict selection in the case of the latter group is emphasized. (3) It is our view that, if liver function is good, this operation is the preferred emergency treatment for persistent bleeding from cesophageal varices. (4) For those alcoholics with significantly impaired liver function, no adequate therapy appears available.
