SUMMARY Ten patients with chronic portal‐systemic encephalopathy have been treated with lactulose and dietary restriction of protein. The treatment was found to be effective in six cases for periods up to 12 months. Two patients with very advanced cirrhosis and hepatic decompensation failed to respond. Two other patients were intolerant of lactulose. It is concluded that lactulose is an effective drug in the treatment of chronic portal‐systemic encephalopathy, and is a useful substitute for neomycin.
